Escalating higher-energy defibrillation regimens (200-300-360 J) are more likely to maintain ventricular fibrillation (VF) termination and restore an organized rhythm compared to fixed low-energy regimens (200-200-200 J). This suggests that higher-energy defibrillation might be preferred in patients with OHCA experiencing shockable rhythms.
Defibrillation requires the delivery of sufficient electrical energy to defibrillate a critical mass of myocardium, abolish the wavefronts of VF and enable restoration of spontaneous synchronised electrical activity in the form of an organised rhythm.
